Currently in a Shelter
If you live in a shelter, the details will be discussed when you meet with your case manager or housing specialist.
At Risk of Becoming Homeless
If you are at risk of becoming homeless, a Homebase counselor will review the eligibility details with you.
If your income has changed or your CityFHEPS rent has increased, you can apply for a modification to your CityFHEPS subsidy.
A fillable version of the CityFHEPS Modification Request form is available online. You will be asked to submit supporting documents with your form. If you need help completing the form, please contact your local Homebase office.

Your CityFHEPS subsidy must be renewed annually. The renewal application is mailed to you five months before the end of your CityFHEPS subsidy year.
You can submit your completed and signed renewal application by email or postal mail. Please be sure to submit supporting documents with your renewal application.
