Owners may need to contact their licensed professional to get a copy of the inspection report and the cashed check.

Certify Removal of Existing Boiler
If a violation is issued for a boiler that no longer exists on the property, the owner must contact a licensed professional (master plumber or oil burner installer) to confirm the boiler does not exist by completing Form OP49 (Self-Certification for Removal of Existing Boiler). The licensed professional must state on the form how the property is being heated.
