The Fire Department only offers copies of Fire Investigation and Fire Marshal Reports by mail. This report reviews the investigation that took place and the results.
Fire Marshal Report requests must include:
- Date of the incident
- Exact address of the location, including borough
- Check or money order for $0.25 (25 cents), payable to "NYC Fire Department" (For "per page" fees, remit payment when you are advised of the exact amount due.)
