The Department of Transportation (DOT) Permits and Customer Service Office is closed for in-person assistance until further notice, but you can contact the agency by phone.

Applications and request forms are accepted by mail. Please do not use a delivery method requiring a signature, because the office is closed and can't accept packages.

The New York State Permit for People with Disabilities is a blue or red plastic hangtag that hangs from the rearview mirror or a vehicle license plate that shows the International Symbol of Access. This permit allows you to park in spaces that are reserved for people with disabilities in off-street lots such as shopping malls. This permit is valid throughout the United States and Canada.

If you’re interested in applying for the State permit, you may also want to consider the New York City Parking Permit for People with Disabilities. It’s a placard that you place on the dashboard that allows you to park at most curbsides, in many "No Parking" and "No Standing" zones on public streets, and at parking meters without depositing money. To apply for the permit, complete the DMV application and include a copy of a valid NYS Driver's License, NYS Non-Driver's Identification Card, or NYC Municipal Identification Card (IDNYC).

You may reapply for the State Parking Permit for People with Disabilities by submitting the DMV application form at least 3 months before your current permit's expiration date to ensure continuous service.

With your competed form, include:

  • A copy of your New York State Driver License, New York State Non-Driver Identification Card, or New York City Identification Card (IDNYC)
  • The medical certification section completed and signed by your doctor
  • The month and expiration date of your current permit next to the permit number on the application form

Sign and date the application form. If you and your doctor don’t provide all of the required information, the incomplete application form will be returned to you.

Renewing vs. Reapplying

The State Parking Permit for Persons with Severe Disabilities (known as the "blue hang tag") is now valid for 5 years from the date it was issued. After 5 years, the permit expires and you must re-apply by submitting a new application to the Department of Transportation (DOT) by mail. DOT no longer sends renewal notices, and you can't renew your permit online.

Permit Not Received

If you have been approved for a Parking Permit for People with Disabilities but never received it, you can report the problem by mail.

Deceased Permit Holders

Permits are no longer valid after the permit holder is deceased. If you are a family member of a deceased permit holder, please return the permit immediately to:

Anyone found using a permit belonging to a deceased permit holder will be subject to fines.

Out-of-State Permits and Meter Waivers

Disability parking permits from outside of New York State aren’t honored for on-street parking in New York City.

Metered parking waivers from municipalities other than NYC are also not honored.

Tickets and Tows

If you have a disability parking permit, were parked in an allowable spot for holders of parking permits for people with disabilities, and received a parking ticket, you can dispute the ticket. To learn more, go to the Parking Ticket or Camera Violation Dispute page.
